Chronic Diseases Program Coordinator
The Chronic Diseases Program Coordinator is responsible for working with the Operations Director for the planning, management, and implementation of the educational programs for adults and children, as well as other outreach activities, related to chronic diseases. This includes the Healthy Hearts program for children (childhood obesity prevention), Por Tu Familia for adults (heart disease prevention), health fairs (health screenings and vaccination), and other community outreach events.
KEY OVERALL RESPONSIBILITIES:
Program Management
- Work with Operations Director in planning/scheduling educational programs and outreach activities, as well as in managing resources and logistics for each of them.
- Prepare and manage paperwork that participants need to complete for each of the classes, events, and outreach activities, including tracking and organization.
- Work in partnership with the Data Analyst to oversee and coordinate data collection and data input with program staff.
- Work in partnership with the Grant Coordinator and Data Analyst to collect and prepare quantitative and qualitative data for grant proposals, grant reports, and executive summaries.
- Collect, record, and maintain financial transactions related to programming.
- Attend NIHHC outreach activities, educational programs, or other community events.
- Monitor community needs and stay current on programming opportunities at the county and state levels. Stay current on research and trends related to chronic diseases.
Outreach Activities, Planning and Management
- Coordinate and attend meetings with community representatives to define schedules of outreach/educational programs (e.g., district or school representatives, church leaders, healthcare providers, etc.).
- Support community relations and the development of programs by participating in community meetings, building partnerships, and creating community awareness of issues related to the programs.
- Work with public relations and the Marketing Coordinator to effectively promote and communicate a strong brand identity for the chronic diseases programs and the organization.
- Support and provide direction for the planning, execution, reporting, and evaluation of all educational programs, health fairs, and outreach activities related to chronic diseases.
QUALIFICATIONS:
- 4-year degree preferred in social work, biology, nursing, human services, public health, or related field.
- At least 3 years of experience in project management and/or program coordination.
- Professional and enthusiastic attitude.
- Proficiency in Microsoft Office Suite (e.g., Excel).
- Familiarity with the use of online (cloud) systems.
- Excellent communication and presentation skills, including verbal and written communication.
- Excellent collaboration skills, with the ability to work as part of a team and with community partners.
- Ability to manage multiple tasks simultaneously and meet project goals and deadlines with minimal supervision.
- Excellent follow-up, problem-solving, and organizational skills.
- Ability to work well with people from all backgrounds with varying degrees of experience.
- Ability to delegate, provide clear instructions, and follow up with team members.
- Availability to work seldom evenings and some weekends.
- Some teaching experience preferred.
- Bilingual in Spanish/English strongly preferred.
